What are your office hours?

We provide Flexible C.A.R.E. and are open everyday including Saturdays & Sundays, by appointment only.

 

How long is an office visit?

Each visit is tailored to the individual based on the specific C.A.R.E. the person needs. Your first visit generally takes about 60 minutes. This time includes consultation, examination and of course, treatment to get you on your way to healing. Follow-up treatments are anywhere between 30 and 45 minutes depending on the complexity of your condition.

 

Do you accept my insurance?

We do NOT participate directly with any insurance plans. All payments are due at the time of service and upon request we can provide you with a bill that you can submit to your insurance carrier.

What forms of payment do you accept?

We accept all major Debit/Credit Cards as well as Zelle and Apple Pay via Square.

 

How much does a Concierge C.A.R.E. visit cost?

All Concierge C.A.R.E. Home Visits are one flat-fee that includes all necessary consultative services & treatments.
Services include: Concierge C.A.R.E.

 

How much does a Telemedicine visit cost?

All Telemedicine visits are one flat-fee that includes all necessary consultative services & treatments.
The fee schedule is as follows: 15 minutes: $100; 30 minutes: $200. 
Services include: Telemedicine

 
Are there forms I should fill out and bring to my appointment?

If you're a new patient, you can electronically complete our Patient Intake Form before your first appointment and email it to Admin@DukeChiroCenter.com.

Are there any additional forms I should fill out if I'm making the appointment for my minor child?

Yes. If you're the parent/guardian of a new patient whom is a minor child, you can electronically complete both our Patient Intake Form & our Consent to Treat a Minor Child Form before your first appointment and email it to Admin@DukeChiroCenter.com.

 

What if I’m a patient of Dr. Duke’s, but it’s been a long time since I’ve seen him? Do I need to complete the full intake paperwork all over again?

No, you do not need to complete the full New Patient Intake Form again, but if it has been more than 6 months since you have seen the doctor, please complete our Patient Intake Update Form form and email it to Admin@DukeChiroCenter.com.

 

Is your practice HIPAA-compliant?

Yes, The Duke Center is fully compliant with HIPAA regulations
